Payment Options

Insured Patients

At the time of admission our receptionist will ask you to sign a claim form and we can then forward your account directly to your Health Insurance Fund. Your account will be inclusive of your accommodation, theatre fees, and any other chargeable items as per Kings Park Day Hospital’s agreement with your health fund.

You will also be required to sign an Informed Financial Consent form. This form will cover the approximate cost including any out of pocket expenses not covered by your health fund. It is the responsibility of the person paying the account to read and understand the information on this form carefully.

Depending on your level of health fund policy you may be required to pay an excess. This will need to paid at the time of admission.

You will also receive separate accounts from your surgeon and anaesthetist for their services.

Self-Funded (uninsured) Patients

Please make sure that you are aware of the cost of admission for our procedure as payment in full will be required at time of admission. We can accept cash, MasterCard, Visa and Eftpos (you may need to verify your daily Eftpos withdrawal limit).

Please note that our fees are not covered by Medicare.

Department of Veterans Affairs Entitled Persons

Kings Park Day Hospital is pleased to be able to provide services to DVA Gold, and where relevant, White Card Holders. All accounts are sent directly to DVA and you will be provided with information regarding the hospital fees on your copy of the Informed Financial Consent form.
